64 POINT SAFETY CHECK PACKAGES
ONE SET PRICE PER MONTH
Package #1
PKG# 1– Consists of 3 Well-check visits per month at 1.5 hour visits
Includes a 64 point safety check
Package #2
PKG# 2– Consists of 4 well-check visits per month at 1.5 hour visits
Includes a “fall risk” assessment with 64 point safety check with a report to family upon completion of all monthly visits.
Package #3
PKG# 3– Consists of 8 well-check visits per month at 1.5 hour visits
Includes “fall risk” assessment with a 64 point safety check, a Basic assessment of daily activity abilities, and non medical Geriatric depression scale assessment sent to family upon completion of monthly visits.
Monthly report sent to family member in charge of care via fax, email, or regular mail at the end of every month.
If there is a concern or danger to the client, the family will be notified prior to end of month report.
